How to Borrow from the Library

The Prevention First library loans materials for two-week periods to Illinois residents over the age of 18, and to cooperating institutions through interlibrary loan.

To apply for borrowing privileges:

  • Stop in at our Chicago library and request an Individual Borrower's Application form. You must present a photo ID/driver's license at the time of application.
  • OR complete our online Individual Borrower’s Application (PLEASE NOTE: If you apply online you must mail or fax a clear photocopy of your driver’s license or state photo ID to: Prevention First, 720 N. Franklin St., Suite 500, Chicago, IL 60610)
  • OR download a PDF copy of the Individual Borrower's Application Form, fill it out completely, and mail or fax to us.

A borrower's card will be mailed to you after receipt of your application form and a copy of your photo ID/driver's license.

To apply for an organizational borrower's card, to be used by anyone in your agency, please contact the library staff in Chicago (312.988.4646 ext 226 or 234).

Please read our Borrower’s Policy before signing the application form:
Patrons may borrow three videos and seven books at a time. All circulating materials, including videos, may be checked out for two weeks. Items may be returned by bringing them to one of our library locations, placing them in one of our library drop-boxes, or sending them to the site where borrowed via insured UPS or mail.

Material may be renewed for another two weeks by calling the library staff on or before the due date. Material may be renewed two times (for a total of six weeks), unless there is a waiting list. Borrowers who need the material at a later time are asked to return it, indicate the date needed, and we will provide it again at that time.

For items that are not returned within one week after the due date, the borrower will be contacted via phone, e-mail and/or mail. Additional materials may be checked out only after the overdue items are returned. Borrowers who do not return material within 30 days beyond the due date are responsible for the replacement cost of the material and will receive an invoice for that amount, plus a $15.00 service charge.

Free Shipping
If you are unable to visit us, we will ship library materials to you free of charge. You will be responsible for the cost of shipping the materials back.

Interlibrary Loan
We strive to provide resources that meet the needs and interests of our patrons. Prevention materials not available from Prevention First can be requested from another library. Materials requested must be related to Prevention First's mission and services. We will attempt to request the materials from free-of-charge libraries. Any interlibrary loan fees are the responsibility of the requesting patron. Requesting patrons must be in good standing. Requests from patrons who have overdue materials will not be processed.
