Consultants' Resources

FY09 Consultant reports for each strategy

  • Monthly Report
    • These reports are to be completed monthly after each phone call and sent to Prevention First staff no later than the 15th day of the following month.
  • Semi-Annual Report
    • These reports are meant to be a review of progress with each agency and to be completed in December and June with agency input. 
    • They should be sent no later than the 15th day of the following month.

    *Note: Will be added July 2008.

  • Consultant invoice
    • This is to be completed with the report and sent to Prevention First staff no later than the 15th day of the following month. 

*Note: The invoice and reports can be completed electronically.

  • Site Visit application
    • This application should be filled out by the agency/consultant to signify the intent to schedule a site visit. 
    • Site visits are helpful when the agency is in need of a strategic planning session or assistance overcoming barriers to implementation or evaluation of an effort.
    • Site visits will be scheduled during the year and completed no later than June 15, 2009.
    • Prevention First staff will review the application and give written notice about an approval or reason for denying the visit no later than 10 business days after submission.

  • Site visit reimbursement schedule
    • This information includes what is usually reimbursed during consultant travel and related expenses.
    • Additional questions should be directed toward Prevention First staff.
