Purposeful Partnership: How Collaboration Across the Continuum of Care Improves Outcomes Recording

This workshop explores how intentional collaboration across healthcare, behavioral health, education, child welfare, recovery support, and community-based systems can improve outcomes for individuals, families, and communities. Grounded in the reported experiences of individuals and families navigating complex systems of care, the session will examine how fragmented communication, siloed services, and disconnected supports often create additional barriers for those already experiencing significant challenges. Participants will explore how cross-sector partnerships can reduce duplication, improve continuity of care, strengthen trust and engagement, and create more responsive, person-centered systems—particularly within rural and underserved communities. Through practical examples, real-world scenarios, and discussion, attendees will learn strategies for building sustainable partnerships that move beyond simple referral relationships toward coordinated, collaborative care.
